How to file a State DOI complaint for a denied pet insurance claim

The single most underused tool in pet insurance: a formal complaint to your State Department of Insurance (DOI). Carriers respond to DOI inquiries within statutory deadlines, and many denials quietly get reversed once a regulator is on the file.

When to escalate to the DOI

File after the carrier issues a final written denial on your first (and, if your policy allows, second) appeal. Most state DOIs will not intervene while an appeal is still pending — they want to see that you tried internal review first.

Step-by-step: filing the complaint

  1. Exhaust the carrier appeal. Submit a written appeal and keep the final denial in writing.
  2. Find your State DOI complaint portal. Every state has one — search "[your state] department of insurance complaint form".
  3. Gather evidence: declarations page, denial letter, your appeal, full vet records covering the claim period, itemized invoices, and a dated timeline.
  4. Write a tight narrative. Quote the exact policy language, state why the denial misapplies it, and name the remedy you want.
  5. Submit online, save the confirmation number, and calendar a 30-day follow-up.
  6. Rebut the carrier's reply. You usually get one chance — keep it to policy language and dated records, not emotion.

  • DOI acknowledges complaint: 5–10 business days.
  • Carrier statutory response window: 15–30 days in most states.
  • DOI closes the file: 60–90 days from filing.

What a good narrative looks like

One page. Three paragraphs: what the policy says, what happened, what you want. Reference exhibits by letter ("Ex. A — Declarations Page, policy language at p.3"). Avoid adjectives. Regulators read hundreds of these and reward clarity.

Common results

The carrier reverses the denial about a third of the time, partially pays in another quarter, and reaffirms the rest — but you walk away with a written, on-the-record explanation that is invaluable if you later sue in small claims court.
